Terms and Conditions for the 8-Week Weight Loss Commitment:

Eligibility: This offer is open exclusively to new patients enrolling in Crafted Sustainable Wellness's weight loss program with semaglutide, who commit to two subsequent months of membership across our categories: Basic, Boost, and Body.

Expected Weight Loss: While the range of healthy weight loss is between 0.5 to 2 pounds per week, our program's benchmark is a minimum loss of 0.5 pounds weekly, amounting to at least 4 pounds over the 8-week period.

8-Week Evaluation Requirements: To be eligible for this commitment's benefits, patients must: Strictly adhere to the prescribed program and achieve less than the set benchmark weight loss of 4 pounds in 8 weeks. Engage in weekly check-ins with their provider. Commit to one weekly "Intervention."

Refund: Refunds of up to $598 may be given. Should patients opt for a refund, it will cover only the basic membership. Refunds will not include costs for additional services, Boost or Body membership upgrades, treatments, or products availed during this period that are not directly tied to the basic membership. 

Switching to Tirzepatide: If a patient wishes to transition to Tirzepatide after not achieving the weight loss benchmark, this can only proceed with the provider's consent. If approved, patients will benefit from a 50% discount on Crafted’s Tirzepatide Upgrade as long as they maintain an unbroken membership with Crafted.

Program Continuation with Tirzepatide: When switching to Tirzepatide, patients must commit for an additional two months, totaling four months on the program. After the two months on Tirzepatide, they can: Continue with Crafted’s Tirzepatide Upgrade at the 50% discounted rate as long as they maintain continual enrollment in at least the basic membership, or opt for a refund of $598. 

Check-ins and Appointments: Patients have an obligation to communicate with their coach at least bi-monthly, either virtually or through in-person visits at our clinic.  

Right to Modify or Cancel: Crafted Sustainable Wellness holds the right to modify or cancel this guarantee anytime without prior notification. Modifications will not affect those already enrolled.

Limitations: This commitment does not cater to any side effects or medical concerns arising from semaglutide or Tirzepatide usage. For potential risks, patients are advised to consult their healthcare professionals.
